The Alzheimer's Drug Discovery Foundation (ADDF) Prevention RFP funds clinical research targeting dementia risk reduction across three distinct categories: precision prevention and risk reduction, combination therapy studies, and comparative effectiveness research. ADDF, founded in 1998 and headquartered in New York, has distributed nearly $400 million to 792 programs in 21 countries and applies a venture-philanthropy model to all its funding. The 2026 prevention cycle is aligned with the 2024 Lancet Commission's identification of 14 modifiable dementia risk factors — including diabetes, hypertension, hearing loss, and depression — and targets both primary prevention populations (no biomarker evidence of pathology) and secondary prevention populations (biomarker evidence without symptoms).

Awards reach up to $5,000,000 for clinical trials; projects run multi-year and cover direct costs only. Co-funding from other agencies or investors is actively encouraged. Eligible applicants worldwide include academic medical centers, universities, nonprofits, and biotechnology companies. Anti-amyloid strategies and cholinesterase inhibitors are excluded. All funded trials must register on ClinicalTrials.gov, and the mission-related investment structure requires successful applicants to negotiate ROI terms — equity, convertible notes, or royalties — tied to scientific and business milestones.

The Letter of Intent deadline for the 2026 cycle is September 14, 2026 at 5:00 PM ET; full proposals are invited thereafter. Competitive proposals will demonstrate how a defined at-risk population, a specific modifiable risk factor, and an intervention mechanism align with the Lancet Commission framework, and will include clear go/no-go decision points to satisfy ADDF's investment-style scientific review process.
